Canoe, Kayak, Paddle Board and Dragon Boat Events

Please complete the following application to the best of your ability. You’ll want to have the following information handy before you start:

  • Information about your event/company/organization like a brochure, newsletter, advertising or website
  • Certificate Request Information – if applicable. You will be required to provide the following for any additional certificates requested:
    • A complete name and address for each entity
    • Certificate holder / additional insured entity names, addresses & any special insurance requirements
  • A copy of the liability waiver you’ll be using
  • For events, a copy of your risk management plan
  • If you are a new client to our agency and have had liability coverage in place elsewhere we will need a 5 year loss run from your prior insurance carrier

Normal processing times may vary – typically 2-3 business days. Incomplete submissions may delay processing.

Coverage is not automatically bound. No coverage has been bound until you receive a certificate of insurance from our office or confirmation in writing from an agent. Premiums are fully earned upon binding. There is no refund for cancellation or under attendance.
